Dr. Stephen J. Clark was raised in Neenah, Wisconsin and has been practicing medicine since 1985. Dr. Clark attended both college and medical school at Northwestern University in Evanston, Illinois and completed his residency at St. Luke's Hospital in Chicago, Illinois.
Lindenhurst Family Practice opened its doors on Falling Waters Boulevard in Lindenhurst, Illinois in 1999. The team at LFP is committed to providing the highest quality of care and strives to empower patients with communication and an understanding of their health and collaborative wellness plan.
Dr Stephen J. Clark has been in practice for over 30 years and is dedicated to serving families throughout various milestones in their lives. Dr. Clark enjoys spending time with his lovely wife, their family, black Labrador Rayna, and is an avid outdoorsman.
Dr. Nader Sallam’s journey in medicine is as diverse and enriching as his approach to patient care. Born and raised in the culturally rich environment of Jordan, Dr. Sallam has always been destined to broaden his horizons. His pursuit of excellence in the field of medicine led him to Egypt, where he completed his medical school education with outstanding dedication and
passion for the discipline. Dr. Sallam's postgraduate journey took him to the bustling city of Staten Island, New York, and later to the community-focused setting of Urbana-Champaign in Illinois, where he completed his residency at the University of Illinois.
His relentless commitment to mastering his craft has allowed him to practice medicine across three continents, offering him a global perspective on health and patient care. Throughout his career, Dr. Sallam has worked both in hospitals and private practices, navigating different healthcare systems and touching the lives of countless individuals along the way. His passion for medicine is evident in his patient-centered philosophy. Dr. Sallam is a staunch advocate for patient empowerment, ensuring that each person's voice is heard and their health needs are addressed on an individual basis. Dr. Sallam finds a special joy in clinic work, as it affords him the opportunity to establish lasting relationships with his patients. He believes that strong connections are the foundation of effective healthcare, leading to better outcomes and a more personalized experience.
Outside of his professional life, Dr. Sallam is a family man at heart. He cherishes the time spent with his wife, children, and their beloved shorkie dog. Whether he's crafting an intricate meal in the kitchen or embarking on adventures to explore new destinations, Dr. Sallam embraces every moment with his loved ones.

Chris completed her undergraduate studies at the University of Chicago, her Master’s at the Illinois Institute of Technology, and her Physician Assistant education at Rosalind Franklin University. She began her career in technology and transitioned to medicine after caring for her parents. She is going into her 14th year with us at Lindenhurst Family Practice and fits in perfectly with our family setting. As a provider she is very knowledgeable, incredibly caring, an excellent listener and very approachable with both staff and patients. She has raised three wonderful children and enjoys spending time with them and her two beautiful grandchildren, and family. Chris is a wonderful practitioner and we are grateful to have her as a team member!

Andy completed his Undergrad at University of Illinois at Champaign-Urbana, with a BS in Kinesiology, went to PA school at Rosalind Franklin School of Medicine and Science and has his MS in Physician Assistant Practice. Andy credits his mom as his biggest motivator for choosing a career in medicine; she worked diligently as a nurse when he was growing up, always going above and beyond to ensure her patients were cared for, even through a 19 year battle with breast cancer. The chance to serve others as she did has been a fitting pursuit in the field of medicine for Andy. Andy’s goals as a practitioner are to provide high level, compassionate, patient-centered care and to encourage patients to be a part of the decision making process, offering knowledge and expertise to create a plan best suited for their needs. Andy is passionate about his own health and fitness and loves Chicago sports teams, especially the Bears and Blackhawks. Andy helped integrate Lindenhurst Family Practice with it's current telemedicine program and expedited that process at the beginning of the COVID 19 pandemic. We are proud of him and grateful he is part of the team at LFP!

Sarah completed her undergraduate studies at Grand Valley State University and her Master's in Physician Assistant Studies at Midwestern University.
Her recent experience includes Illinois Bone and Joint and Advocate Condell Hospital. Sarah's credentials and certificates include Illinois and Wisconsin State PA-C licensure, she is NCCPA board certified, CSL and DEA licensure, basic life support and EMT-B.
