Frequently Asked Questions

What do I need to bring to my appointment?

Please bring a current ID and current insurance card, filled out paperwork (for new patients, existing patients need to re-register every year), and a list of your current medications/dosages (or as best as you can remember).

When should I arrive for my appointment?

Please arrive approximately 10-15 minutes early to your appointment to ensure all pertinent information is prepared and you get the most out of your time with us. 

Do I have to pay for my visit at the time of my appointment?

Most insurances charge a copay at the time of your visit. If you do not have insurance or your insurance is not covered at our location, payment will be due at the time of your visit. Check the 'Participating Insurance Carriers' section of our website for a full list of participating insurance carriers, verify with your insurance carrier personally, or call our front desk at (847)356-9300 with questions.

Who do I contact if I am sick after hours?

If it is an emergency, please call 9-1-1. If it is an urgent non-emergency, after hours, and you are a current patient, please call the doctor on call (224)406-7791. Please know in advance which hospital is in your insurance plan; NSU-Highland Park, or Lake Forest. For after hours, patients that call the doctor on call may be charged a telephone visit for medical advice.

How do I refill my prescriptions?

*for certain refills, you MUST to be seen for a med check by a provider, this is standard office policy* Have your pharmacy fax your refill request to our office (847)356-7260; please allow 48 hours for processing.

What is the age range for new patients?

We accept new patients between the ages of newborn and 59 years of age. We recommend internists after the age of 59 to ensure the continuation of ideal and specialized care for individual needs.
